- Contract holder
- The person or organization named as holder in the provider’s contract records.
- Supply reference
- An identifier used to locate a particular supply or service; its format depends on the provider.
- Requested date
- The date you ask the provider to apply, pending its confirmation.
- Handover record
- A dated record of readings, equipment or other details observed when a service or property changes hands.
- Effective date
- The date the provider confirms for the applied change, which should be checked against the request.
